## Description
Super Admins can now record a category and reason when suspending an
account, review the complete suspension history on the account details
page, and correct the latest suspension metadata without losing its
original timestamp. Suspension events are stored internally on the
account without changing customer-facing account API payloads.

## What changed
- Require a suspension category and a reason of up to 256 characters
when an active account is suspended.
- Store append-only suspension events in `accounts.internal_attributes`,
while preserving unrelated internal metadata.
- Allow corrections to the latest event for an already suspended account
without changing its timestamp.
- Show the full suspension history, newest first, on the Super Admin
account details page.
- Add visual dividers between top-level sections on the Super Admin
account edit page.
- Keep legacy suspended-account edits and new-account creation behavior
unchanged.
## How to test
1. Open an active account in Super Admin and choose **Suspended**.
2. Confirm the category and reason controls appear, reject incomplete or
invalid values, and enforce the 256-character reason limit.
3. Suspend the account with each supported category and confirm the
event appears on the details page.
4. Reactivate and suspend the account again; confirm prior history is
retained and a new event is added.
5. Edit a suspended account's latest category or reason; confirm its
original timestamp is preserved.
6. Confirm a legacy suspended account without history can still be
edited without supplying suspension metadata.

On self-hosted instances without email configured, users created from
Super Admin can get stuck in an unconfirmed state. This PR implements
the default at the Super Admin frontend form layer, not in backend
creation logic.
What changed:
- Added a custom `ConfirmedAtField` for Super Admin user forms.
- Prefills `confirmed_at` with current time on the **New User** form
(`GET /super_admin/users/new`).
- Kept backend create behavior unchanged
(`resource_class.new(resource_params)`), so API/manual payloads still
behave normally.
Behavior:
- In Super Admin UI, `confirmed_at` is prefilled by default.
- If someone wants an unconfirmed user, they can clear the
`confirmed_at` field before saving.
- If `confirmed_at` is omitted from payload entirely, the created user
remains unconfirmed.
Scope note: external signup flows are intentionally unchanged in this PR
(`/api/v1/accounts`, `/api/v2/accounts`, and social/omniauth signup
behavior are not modified).

This pull request introduces several changes to implement and manage
usage limits for the Captain AI service. The key changes include adding
configuration for plan limits, updating error messages, modifying
controllers and models to handle usage limits, and updating tests to
ensure the new functionality works correctly.

These changes ensure that the Captain AI service operates within the defined usage limits for different subscription plans, providing appropriate error messages and handling when limits are exceeded.
* feat: installation config in super admin console
* Added the ability for super admins to view, edit and update
installation config values. Also they can add new isntallation config
values. The impact of editing and adding depends on which all
installation config values are being used in the code.
* Known limitation now: Ability to edit hash values (for eg: feature
flags) are disabled. This requires more work and will be taken up in
a secondary set of changes.
